What Is Ankle Sprain?

An ankle sprain occurs when the ligaments that stabilize the ankle are overstretched or torn, most often when the foot rolls inward ("inversion"). Sprains are graded by severity, from mild overstretching to a more significant partial or complete tear.

Swelling and pain alone do not establish the grade of sprain. Repeated sprains can leave deficits in balance and strength even when swelling settles. Rehabilitation and a graduated return to activity remain central.

Only when weight bearing and balance work are appropriate for your injury. Early unsupported single-leg work may be unsafe. Follow the protection and rehabilitation plan you were given.

Persistent instability may reflect ligament injury or strength and balance deficits. It warrants assessment rather than repeatedly treating pain alone.

Significant trauma, inability to bear weight, deformity or concerning bone tenderness should be assessed medically first. Acupuncture cannot rule out a fracture.

Not always, but a sprain that cannot bear weight, is very tender over the ankle bones, or is severely swollen should be evaluated to rule out a fracture. When in doubt, get it checked before loading it.

Repeated sprains usually reflect residual weakness and reduced balance control from incomplete rehabilitation. Balance and strengthening work — see the exercises above — is the best-supported way to break that cycle.

In the first days, protect it, rest, and use ice, compression, and elevation to manage swelling; begin gentle pain-free motion early, and progress strength and balance as it settles.
